APPLICATIONS:
Applications
must be received by January 12, 2012 (after this date, call
or email for availability).

ATTENDANCE:
The Massachusetts Office of EMS
requires 100% attendance. If you miss a class it MUST be made
up. Make ups may consist of you traveling to another EMT course
to sit in on missed lectures, or meeting with the instructor on
additional dates, or attending a scheduled make up session.
PAYMENT SCHEDULE:
1. DO NOT send any money with the application.
2. Once you have been accepted
into the course you will be required to send in the registration
fee of $350.00. This fee is non-refundable (unless the course
is canceled).
3. On the first day of class $350.00 is due
4. On the seventh class $350.00 is due

SMOKING:
Under state law, there is no smoking.
REMEMBER:
Do not
